We provide home care by focusing on the strengths and priorities of our service users. We encourage them to draw on their own resources, build resilience, and achieve the desired outcomes. Our support extends to helping them maintain and develop rewarding social and family relationships, as well as fostering meaningful connections with other individuals, groups, and universal services within their communities. We collaborate with partners to nurture and grow community groups, encouraging them to maximise their ability to support people.
7CCS involves service users in their own health and care services, shifting our core focus from "What is the matter with you?" to "What matters to you?" This approach builds inclusive conversations between staff, service users, and their families, identifying needs and setting goals that truly matter to them.
By prioritising what’s important to our service users, we create a better and more personalised care experience, fostering a sense of ownership and connection that enhances their well-being and quality of life.
7CC’s services will be providing all personal care tasks to people between the ages of 18-64 and 65+, who will often require care due to old age frailty. Under the umbrella term of personal care, we will be assisting people with personal hygiene, administration of medicines, washing and dressing, oral care, skin care, moving and handling, feeding, and continence care. To supplement this offering and to ensure that we meet the complete needs of individuals, we will be supporting people with non-personal care tasks that enhance their quality of life, such as shopping cleaning, meal preparation and companionship. We aim to ensure that people are looked after in a way that protects their safety and maintains their independence while improving their overall well-being. The care we deliver will be completely person-centred and only delivered after thorough assessments have been conducted with our service users, involving them and if necessary, their families completely in their care. We are focused and committed to the ongoing improvement of our service and will employ several quality assurance mechanisms to ensure that this is possible. We will work with our local council as well as the CQC and external stakeholders to support the community most seamlessly and effectively and will be seeking feedback from several channels in addition to of course our service users to ensure that our service is responsive to changing needs of those we serve.
